The outcomes of pelvic fracture urethral injuries stratified by urethral injury severity: a prospective multi-institutional genitourinary trauma study (MiGUTS). Urology. 2022;170:197–202. The authors of this study looked to provide some of the first evidence stratifying PFUI outcomes based on severity of injury. This prospective study found that a simplified classification system regarding severity of PFUI (complete, minor, and partial injury) gave insight into which patients would need delayed urethroplasty versus close observation.

New imaging modalities, in addition to the standard-of-care RUG, are being explored in the detection and management of PFUI. These authors found that MRI may better delineate tissue planes and aids in surgical planning of delayed urethroplasty and whether ancillary techniques may be necessary for definitive repair.

A retrospective look at term outcomes after definitive surgical repair for traumatic pelvic fracture urethral injuries—does initial management make a difference? Urology. 2022;160:203–9. While the initial management of PFUI has been controversial, these authors performed a retrospective study to determine whether primary realignment versus suprapubic tube placement impacted long-term outcomes. They found that there was no significant difference in long-term outcomes in patients after delayed urethroplasty who were initially treated with primary realignment or suprapubic tube placement.
