Ethical approval and consent to participate

All subjects gave their informed consent for inclusion before they participated in the study. The study was conducted in accordance with the Declaration of Helsinki of 1975, as revised in 2000 and 2008, concerning human and animal rights, and the authors followed the policy concerning informed consent as shown on Springer.com.

Consent for publication

An informed written consent was obtained from the patient after full explanation regarding her images being published for academic interest. The patient did not have any objection regarding use of her images and gave due permission to use them.
