Human rights statement and informed consent

All procedures were conducted in accordance with the ethical standards of the responsible committee on human experimentation (institutional and national) and with the Helsinki Declaration of 1964 and its later amendments. This retrospective study received approval from the ethics committee of our institution. Given the retrospective study design, the requirement for informed consent was waived.
