Devlin et al. [1] observed a faster resolution of delirium tremens (DT) with an alpha-2 agonist, clonidine, compared to neuroleptics (haloperidol, quetiapine, or both). Combining drugs acting through different sets of receptors presents beneficial effects [2], such as synergism. Therefore, the next trial should compare alpha-2 agonists versus neuroleptics versus a combination (e.g., clonidine + haloperidol).
